Housing Reservation Forms

You cannot register for housing for CHI '95 via the web or email!!

You must mail or fax your Housing Form. You can get the Housing Form here on the web, print it, fill it out, and then mail/fax it in.

The Housing Form is avilable in three formats:

All three versions of the Housing Form contain instructions on where to send it and what the deadlines are. Housing Information is available which lists locations, costs and features of the six conference hotels.

Also, do not forget to send in your Conference Registration Form.
